Prior instrumentation did not reach full length. Gained 2mm to clean further. Patient to return in 6 months to verify appropriate bone healing.

Apicoectomy of number 19 of Extruded Broken File
Root canal had broken file extending out of the apex. Root end surgery was completed to remove broken file and root end granulation tissue. Distal canal also had a broken file but since there was not periapical radiolucency around the distal root, it was left alone and put to monitor.

Apical Perforation and Periapical Radiolucency of number 19
Patient came in with an apical perforation and periapical radiolucency at the apex of mesial. Root end surgery was completed on the mesial roots and bone graft was added. 6 months later, the surgical site still looks good and patient was out of discomfort. Broken File Bypass number 3
Patient presented with broken file in the mid canal of mesial canal. The broken file was bypassed and all 4 canals were found and cleaned. Patient got a crown over tooth number 3 and reported tooth is doing well.
